He fucks it up and gives the signal to his second. His second fucks up the beheading and tries to commit seppuku in shame. Then his second fucks that up...
The whole time the stream is glitching and stuttering, so you kinda see this play out in a set of freeze-framed vignettes. And then there's just a big splash of blood over the camera lens as the guy running the feed commits seppuku, and it cuts to black.
