Yeah. It seems like what's happening is that when one replies to a comment it gets marked as read (for obvious reasons) but this also happens if anyone else replies to it too and thus it gets removed from the inbox incorrectly.

I'm pretty confident this is at least one cause, because every reply to my posts or comments that hasn't shown up in my inbox was one that got replied to by someone else before I saw it. Comments with links to youtube, twitter, or reddit suffer the most from this because the bot replies to those with mirrors/different frontends nearly immediately, stopping them from ever going to the inbox.

I noticed it most with federated users that were subjected to the mass line of dunk culture and I thought it had something to do with other instances, but this makes a lot more sense given that comrades were hitting them hard and fast so I never saw the notifications at all. Two missing inbox replies from other hexbears this afternoon are there in my 'all' feed and have replies to them from others, I think you've nailed it.
