You can also pick your genitalia independently of your body type.

There's an option when you first start the game to enable "cinematic nudity".
If you toggle it on, there are some scenes where the protagonist is naked and a handful of sex scenes that you have to go out of your way to encounter.
All the genitalia option during character creation does is pick what's on display during these scenes.

It is sort of weird that it takes 3 romantic suggestions to be able to turn down Gale but at the same time the one scenr you see people griping about (the magic scene) which asks you if you want to imagine kissing him also asks if you want to show him an image of you ripping off his head. It's clear they are uncomfortable with their just being options listed for gay romance.
I played a woman and Gale gave me huge creep vibes. I kept politely turning him down, until he isolates me on some grass field to tell me he loves me?? Fuck off dude. I've clearly shown you I'm not interested.
Next time we interact he says he's "glad to be with me, though this wasn't the way he imagined"???? Can you shut the fuck up please you incel friendzone loser.
